Effects of Fractionated Intracarotid ?????????????


Abstract
Many substances in blood are unable to penetrate into the brain tissue across the cerebral vascular wall, the phenomenon termed as the blood-brain barrier (BBB). The iodinated contrast media (ICM) are known to disrupt BBB due to high osmolarity
and
chemotoxicity. In the present animal experimentation, authors investigated the brain-tissue uptake of 99mTcO4 and 125I-HAS in 52 rats to assess the effect of intracarotid injection of several commonly used ICM including iotrolan(nonionic dimer),
iopromide (nonionic monomer), and meglumine ioglicate (ionic monomer) and also the protective effect of fractionated intracarotid ICM injection and steroid premedication on BBB.
In addition, to assess the additive damaging effect of nonionic dimer on BBB in acute hypertension, we evaluate the uptake of 99mTcO4 and vascular leakage of Evans blue (histofluorescent microscopic examimation) after intracarotid injection of
iotrolan
and saline in 12 rats, in which acute hypertension was induced by intravenous injection of metaraminol.
@ES The results were as follows :
@EN 1. No significant difference of 99mTcO4 and 125I-HAS uptake was noted among the saline, iotrolan, and iopromide groups, but the uptake of both agents increased significantly in the meglumine ioglicate group as compared with the saline group
(P=0.001
and P=0.008, respectively).
2. No significant difference of 99mTcO4 and 125I-HAS uptake was noted between groups of continuous and two fractionated injection, but those in four fractionated injection group were significantly low as compared with those of the continuous
injection
group (P=0.026 and P=0.006, respectively).
3. The brain uptake of 99mTcO4 and 125I-HAS in the group of meglumine ioglicate injection with methylprednisolone premedication was significantly low as compared with that of meglumine ioglicate injection group without methylprednisolone
premedication
(P=0.007 and P-0.012, respectively).
4. The bfrain uptake of 99mTcO4 in the hypertensive group of inotolan was slightly higher than that in the normotensive group of iotrolan injection (P=0.02) and that in the hypertensive group of saline injection (P=0.032).
In conclusion, nonionic monomer ICM (iopromide) and a dimer (iotrolan) have little detectable of fect of BBB whereas ionic ICM (meglumin ioglicate) has a definite damaging effect. The effects of ionic meglumine ioglicate on BBB can be
significantly
reduced by the fractionation of ICM injection as well as the steroid premedication. Acute drug-induced hypertension potentiates BBB damaging effect of dimeric as well as monomeric nonionic ICM.
